Your organisation's (or auspice organisation's) recent Financial Statement (income statement or balance sheet) - audited if appropriate

Your organisation's (or auspice organisation's) current Public Liability Insurance Certificate of Currency (must be $20,000,000)

Your organisation's (or auspice organisation's) ABN (if relevant)

The budget income and expenditure details for the project or equipment purchase (Section 5)

Quotes for expenditure items more than $1,000 (Section 5)

An authorised representative of your organisation to complete the declaration. (Section 6)
