Your organisation or auspice current public liability insurance certificate of currency (Section 2)
Annual financial statement (income statement or balance sheet) for your organisation or auspice, (audited if appropriate) (Section 2)
Other documentation to support your proposal, including letters of support (Section 4)
Budget income and expenditure details for your proposed activity (Section 5)
Quotes for all expenditure items more than $2,000 (Section 5)
Applicant Conflict of Interest declaration (Section 6)
An authorised representative of you organisation has to complete the declaration (Section 6)
