To complete this application, you will require the following documents and information listed in the checklist below.
- Your organisation or auspice ABN (Section 2)
- Your organisation or auspice current public liability insurance certificate of currency (Section 2)
- Annual financial statement (income statement or balance sheet) for your organisation or auspice, (audited if appropriate) (Section 2)
- Other documentation to support your proposal, including letters of support (Section 4)
- Budget income and expenditure details for your proposed activity (Section 5)
- Quotes for all expenditure items more than $2,000 (Section 5)
- Applicant Conflict of Interest declaration (Section 6)
- An authorised representative of you organisation has to complete the declaration (Section 6)
Without all the documents and information listed above, your application cannot proceed to the assessment stage.
